CLARK UNIVERSITY SPORTS MEDICINE
The sports medicine staff, in conjunction with its facilities, will provide professional allied health care coverage to all sponsored varsity intercollegiate athletic teams, throughout the entire school year. The sports medicine staff at Clark University is comprised of two full-time athletic trainers, and one split appointment athletic trainer/strength and conditioning coach. It is the goal of the staff to provide health care coverage with equal access to prevention, evaluation, treatment, and rehabilitation services of athletic injuries for all student-athletes while competing for Clark University.

MEDICAL CLEARANCE
For New Student-Athletes
Please complete documents online and scan and upload a copy of your physical. You may register or access your ATS profile at clarku2.atsusers.com.
All paperwork is due by the Friday of the first full week of August.
Instructions for registering with ATS and completing forms. (New Students)
Pre-Participation Evaluation (must be within 6 months before your first day of practice and ONLY this form will be accepted!)
Your physical may not be older than 6 months before your first day of practice. General start dates are the end of August for fall sports, the beginning of September for all others.

- Sickle Cell Trait Waiver
- Concussion Policy Form
- Assumption of Risk
- Insurance Coverage Agreement
- Release of Medical and Treatment Information
- Insurance Information & Picture of Card
- Emergency Contact Information
- ADHD Form (only if taking applicable medication)
- Doctor's clearance notes for any unresolved injuries or summer surgeries.
For Returning Student-Athletes

Instructions for Completing ATS Documents (Returners)
Necessary Paperwork to be Completed on ATS:
- Annual Medical History
- Sickle Cell Trait Waiver

- Release of Medical and Treatment Information
- Check insurance and contact information for accuracy
- Doctor's clearance notes from any injuries that were not resolved during the previous school year or summer surgeries.
Inclement Weather Policy and Procedure
It is vital for the safety of our student-athletes, coaches, staff, and spectators that the Inclement Weather Policy and procedure be implemented. Mandatory compliance with this policy and procedure must be met to ensure the safety of all involved. Hours of Operation During School Year
Hours of operation will vary depending on the practice/game schedule. Hours are posted outside of the Kneller Athletic Center Athletic Training Room.
The athletic trainer covering the practice will be available at the closest athletic training room to the practice 1 hour before it begins.
When there are no practices occurring, the Kneller Athletic Training Room will be staffed for treatment/evaluation appointments.
General Hours of Operation:
Kneller Athletic Training Room: Weekdays - 10 am-4 pm
Dolan Athletic Training Room: As Needed
Weekends: Event/Practice Coverage only.
Visiting Team Information
Athletic Trainer Coverage
An Athletic Trainer is available at all home contests involving Clark University student-athletes.
Please call in advance if your team will be traveling without an AT. We will be happy to help with pre-game taping, but require a note or phone call with specific requests; treatment will be limited to heat and ice.
If an AT is traveling then they will be able to use modalities such as hydrocollator packs, whirlpools, ultrasound, and electric stimulation.
During home contests, water and ice will be provided and an AED and splint kit will be available.
Crutches will be loaned to injured visiting student-athletes on an as-need basis.
Generally, an AT does not travel with our teams with the exception of postseason and tournament play. Please note that each team's emergency medical information can be found in the medical kit provided.
